To provide a safe, responsive, efficient, timely, high quality evidence based nursing service to children and young people with learning disabilities, their families and carers. To be aware of, and participate in meeting the psychological, social, physical and spiritual needs of children and young people. Undertake a range of comprehensive assessments on children and young people with highly complex and challenging needs. Develop a range of person centred interventions and specialised care plans. To coordinate, monitor and evaluate highly complex interventions and packages of care. Respond appropriately and flexibly to unpredictable situations. To facilitate children and young peoples and their carers understanding of highly complex issues in relation to their needs, considering significant communication impairments and their level of understanding. Work in collaboration with other agencies to promote, identify and facilitate the use of reasonable adjustments to ensure health needs are met and health outcomes are optimised. To embrace public health role, promoting health and wellbeing and where possible, prevent ill health. To undertake screening and triage assessments for newly referred children and young people. Identify, manage and minimise risk If successful, this vacancy will require an enhanced DBS check at a cost of £54.50. This cost will be deducted from your first Trust salary.
