Job summary Under the direction of theLead Care of Older People and Stroke Pharmacist in conjunction with the Pharmacy Lead for Clinical Services at Ealing Hospital, the post holder will: Provide an enhanced clinical pharmacy service to the Same Day Emergency Care (SDEC)-Frailty (Ealing) and Older People wards SDEC-Frailty Pharmacy service is a referral based enhanced service for complex medicines optimisation and review To work as part of the SDEC-Frailty team and wider Older People teams and liaise with community-based services to ensure the seamless transfer of patient care on discharge. Provide daily ward pharmacy service to Care of Older people wards and SDEC-Frailty team when referrals are made Main duties of the job o To lead and develop the specialist pharmacy service to the acute geriatric wards and services (SDEC-Frailty) and where applicable, under the direction of Lead Pharmacist for Older People and Stroke for the Trust, be involved in Virtual Ward and /or HospitalHome service, when set up. o To provide highly specialised advice on all medicine related issues in area of responsibility i.e, cost effective optimal treatment either for individual patients or groups of patients in line with local, regional and national guidance and policies o Provides high level of pharmaceutical care to patients on the ward, obtaining accurate medication history, reviewing choice of treatment and dosage to ensure safe and effective use of medicines in individual patients in line with Trust guidelines and Formulary.
