Job Description
Duties:
* Assess and determine supportive services to meet each client's unique needs
* Coordinate necessary services based on needs assessment such as assistance with housing, funding for treatment, health insurance, employment
* Assist clients in implementing a daily routine through all stages of recovery
* Provide education, screening, and referral for communicable diseases
* Act as a liaison between client and other providers assisting in client care
* Accurately document interactions and sessions with clients
Experience:
* Previous experience in a similar role preferred
* Strong organizational skills and attention to detail
* Proficient in Microsoft Office Suite (Word, Excel, Outlook)
* Excellent written and verbal communication skills
* Ability to multitask and prioritize tasks effectively
* Strong professional boundaries
