The team is a countywide team which is centrally based in Droitwich with satellite bases in Evesham, Bromsgrove and Kidderminster. This post is primarily based in Droitwich covering South Worcestershire. They visit patients in their own home and support them in remaining at home and preventing admission to hospital, and supporting early discharge from hospital.
The post is all worked as unsocial hours hence picking up pay enhancements for all hours worked which makes this post more attractive. More money for less hours!

Main duties of the job
As a Community Nurse working with the Evening Team, most of the work allocated to you will be of an unplanned nature. The service works as an on-call service covering the County of Worcestershire taking direct referrals from patients, 111, Ambulance Service, and discharging hospitals. You can expect to have a comprehensive induction and work alongside peers until you are confident to then work alone.
The evening service nurses primarily work alone and have work allocated via a handheld tablet by the triage nurse based at Droitwich. You will be able to start your visits from home if the geography dictates.
The patients that are seen during the evening and night vary immensely and no 2 shifts are the same. For example, we see catheters, palliative care, wound care, administration of medication, diabetics, and patients requiring intensive support rehabilitation following discharge from hospital or admission prevention. As a team, we often undertake initial assessments of patients in the community.
As part of a new initiative, we are working closely with West Midlands ambulance service and 111 and assist in responding to urgent care response. These can be patients that have fallen or patients requiring further assessment to aid prevention in admission to hospital.

You will be joining the Evening element of one of our dynamic and supportive Neighbourhood Teams to provide a 24-hour admission avoidance service to the population of Worcestershire which encompasses both urban and rural areas.
The successful candidate will deliver independently a high standard of nursing care for patients in their own homes, ensuring that care provided maintains the individual's privacy and dignity, values diversity, and respects cultural and individual rights and choices.
We are looking for candidates with good communication skills, a flexible approach, and enthusiasm to work within the team. You should be a committed, highly motivated, and forward-thinking individual who is a good team player.
The successful candidate will also assist the team to develop and achieve team and organizational objectives. They will also participate in staff meetings and training & development activities as necessary to ensure their skills and knowledge are up to date.
Due to the nature of the post, the applicant must be able to travel independently and have access to a suitable vehicle for business purposes, and hold a valid UK driving licence. Whilst a base will be identified, staff can be asked to work anywhere within the Business Unit or Service if necessary.
