Hales Home Care is looking for experienced Support Workers / Care Workers / Care Assistants to join an exciting new service in Norwich/East Norfolk as Reablement Intervention Care Team Members .
8-hour or 12-hour shifts available.
To be successful in this Reablement Support Worker role, you must:
1. Be available to work 7 days per week on a 4 on/3 off shift pattern
2. Have a full driving licence and access to your own vehicle
3. Have previous experience in providing person-centred care
About the Reablement Support Worker role
The role will be working in teams of two to support rapid hospital discharges and provide the care and support necessary to enable people to regain independence and achieve their goals for independent living. This will involve dynamic risk assessment and an outcomes focussed attitude to care delivery.
You will be required to liaise with other health professionals, and signpost service users and family members to the appropriate additional support, equipment and services to aid their ongoing recovery and long-term care needs.
You will be an experienced care & support worker, ideally qualified to Level 3 or equivalent with a desire to develop your role.
