40 Hours per Week (Monday thru Friday schedule) - 100% Outpatient only

As a primary care provider, you will:
1. Build and follow your own primary care panel of patients
2. Practice and contribute to the development of evidence based medicine
3. Provide continuity of care to clients by establishing an ongoing primary care/client relationship with individuals and families
4. Participate in quality improvement initiatives and educational enrichment such as Grand Rounds
5. Be part of a multi-disciplinary practice that offers a blend of individual and population based care
6. Be supported by a strong team of Medical Assistants and RN’s and work in an innovative practice environment utilizing Advanced Access scheduling
7. Utilize a Meaningful Use (MU) certified electronic health platform to document and manage patient information and communicate with other team members
8. Assess, diagnose and manage acute and chronic health problems using the models of planned care and the chronic care model
9. Perform comprehensive health histories and physical assessments developing appropriate treatment plans
10. Work in an interdisciplinary care model with co-located behavioral health, nutritionist, dental, and other ancillary care providers
11. Participate in after hours call no more than one in four nights and weekends
