An exciting opportunity has risen to join the Cardiac Rehabilitation team at University Hospitals of Leicester NHS Trust. We are seeking a Part time Band 3 Cardiac Rehabilitation assistant for our services.
We are looking for someone who has experience in health and social care with a keen interest in exercise training for health or fitness and can speak Gujarati and at least one of the following languages; Hindi, Urdu or Punjabi to support our South Asian Population.
Cardiac Rehabilitation is delivered across 3 sites from Glenfield Hospital, Leicester General Hospital and the National Centre of Sports and Exercise Medicine (NCSEM) at Loughborough University as well as covering other non UHL sites within the Community. This person will be required to work flexibly across all sites and therefore a clean driving licence is desirable.
You will be responsible for delivering essential patient care and reviews under the supervision of a qualified practitioner. Excellent communication and interpersonal skills are essential. You will be an active member of the multi-disciplinary team composed of Nurses, Physiotherapists and Exercise Physiologists, supporting patients and promoting recovery through planned programmes of care with the aim of preventing unnecessary hospital admissions and facilitating patients’ effective self-management and rehabilitation.
You will need to treat all patients with respect and courtesy, maintaining dignity and respecting individual needs and differences, both physical and cultural.

Service Delivery & Development
1. To assist the health professionals in patient assessments and care throughout all stages of Cardiac Rehabilitation, predominantly for South Asian speaking patients (with limited or no English) and also support English speaking patients.
2. Respecting people’s dignity, wishes and beliefs, involving them in shared decision making, and obtaining their consent for care to be undertaken, within the constraints of Trust policy.
3. Work in partnership with patients and service users to promote health and well-being in line with national and local policies. The support and monitoring of patients throughout, enabling them to address their own health and wellbeing as far as it is possible for them to do so.
4. Promptly alerting the relevant person when there are unexpected changes or identified risks to individuals’ health and wellbeing.
5. The provision of information to the multidisciplinary team on how individuals’ needs are changing.
6. To accept responsibility for a delegated workload, taking an active role in promoting independence and meeting the needs of the individual.
7. Maintain effective communication with patients, relatives and members of the multi-disciplinary team regarding all aspects of care demonstrating a range of appropriate communication skills.

9. Monitor and record observations such as blood pressure, heart rate, and exercise exertion, height and weight/waist measurements.
Contribute to Cardiac Rehabilitation records and assist with the collection
Governance, reduction of risk, audit and research
Use health related information only for the purposes of what it is intended for and in accordance with the Data Protection act.
Ensure confidentiality is maintained at all times and information relating to patient / users and personnel is used only in connection with authorised duties.

Undertake only those duties and delegated tasks and treatments within own limitations, level of competence, skill and training.
Deliver direct and indirect care within Directorate, Trust, local and nationally agreed protocols and policies.
Assist with the collection of simple data for audit/research and other purposes as delegated by the Pulmonary Rehabilitation team.
Contribute to the management of risk through risk assessment processes to ensure that risks are reduced at all times to the lowest minimal levels.
Adheres to Trust policies, procedures and guidelines and promotes the compliance of others.
Patient/Customer Service
Beresponsible for ensuring that consistently high standards of customer service are delivered.
Maintain healthy working relationships with the Multi Professional Team, patient and users.
Support patients and users in maintaining their identity and relationships.
Ensure that lines of reporting to the Senior Cardiac Rehabilitation Specialists are timely, robust and efficient.
Take responsibility for undertaking your own mandatory training and updates as required.
Complete specialist training required that allows Health Care to be delivered to the user through new ways of working.
Take responsibility in collaboration with line manager within the appraisal / Personal Development Planning (PDP) process for identifying own learning and development needs within the knowledge and skills framework (KSF).
