As an ACP,you will play a crucial role in assessing, diagnosing, and managing a varietyof clinical cases, particularly for patients requiring urgent care. You willwork independently and collaboratively with our multidisciplinary team,contributing to the enhancement of patient care across Worcester City.

Key Responsibilities:
1. Provide advanced clinical assessment, diagnosis, and treatment to patients within the Acute Visiting Service.
2. Work autonomously to manage complex or undifferentiated cases, ensuring high standards of care.
3. Support the development and implementation of care pathways aimed at reducing hospital admissions.
4. Prescribe and review medication, ensuring it aligns with patient needs and evidence-based practice.
5. Collaborate with other healthcare professionals to deliver comprehensive, patient-centred care.
6. Participate in the improvement of clinical services through audit, research, and service development.

Person Specification
Qualifications
Essential
7. Degree Level education, NMC or HCPC registered.
8. Must hold a digital badge or be able to work towards/apply for one.
9. Masters diploma in Advanced Clinical Practice or Health Assessment or equivalent post-graduate study Level 7.
10. MSc Advanced Practice or willingness to complete MSc Advanced Practice
11. Independent Prescriber (or PGD qualification for Professions without prescribing within professional competencies).
12. CPD Portfolio to demonstrate learning and development.
Desirable
13. Leadership course.
14. Teaching or Assessment and mentoring qualification.
Experience
Essential
15. Significant relevant post-registration experience within the speciality.
16. Competent in the planning and managing of complete clinical episodes of care for all patients; working autonomously.
17. Experience of teaching at an informal and formal level.
18. Able to role model and lead whilst working independently and within a team.
19. Participated in research, audit and developed service improvement strategies.
