Are you passionate about patient care and ensuring efficient hospital operations? Join our team as a Patient Flow Coordinator and play a vital role in optimizing patient discharge processes within our Trust.
In this role, you will provide comprehensive support to facilitate timely discharge of patients who no longer require acute in-hospital care. You will collaborate closely with ward staff to address and overcome barriers and delays in patient care. Additionally, you will support the discharge teams and site teams, including site managers and capacity coordinators, to ensure a smooth patient journey through the inpatient system.
We offer a dynamic and supportive working environment, with opportunities for professional growth and development. This is your chance to make a significant impact on patient care and hospital efficiency.
To be successful in this role, you should possess strong organisational and communication skills, and be able to work effectively in a multidisciplinary team. Experience in healthcare operations or a related field is preferred, along with a proactive approach to problem-solving and process improvement.
A key aspect of this position involves maintaining a real-time understanding of capacity and demand, coordinating patient flow operations with precision and efficiency. You will be responsible for implementing best practice flow methodologies, such as Red to Green principles, effective ward and board rounds, and criteria to reside. Your efforts will be crucial in ensuring that patient flow is efficient and meets the highest standards of care.
Working collaboratively with multidisciplinary teams across the organization, you will support the wider Capacity and Flow Clinical Business Unit (CBU) in managing relationships with stakeholders involved in site operations. Your ability to foster strong working relationships will be essential in achieving our goal of optimizing patient flow and hospital capacity.
We are dedicated to fostering diversity and inclusivity within our workforce and warmly welcome applications from individuals of all backgrounds, with a particular emphasis on encouraging those from disabled, Black, Asian and Minority Ethnic (BAME) communities, as well as candidates from our local area.
