Job Description:
Devon Partnership NHS Trust is seeking a passionate and dedicated Band 5 Mental Health Practitioner to join our vibrant and supportive team in Tiverton.

As a Mental Health Practitioner, you will provide high-quality care and support to individuals experiencing mental health challenges within our community services.
What We Are Looking For:
* A Registered Mental Health Nurse (RMN), Occupational Therapist (OT), or Social Worker with a passion for supporting individuals with mental health needs.
* Strong communication, organisational, and time-management skills.
* The ability to work effectively as part of a multi-disciplinary team.
* Experience in mental health or community settings is desirable but not essential.
* A proactive approach to continuous learning and development.

Main duties of the job
Key Responsibilities:
* Provide assessment, treatment, and support for individuals with mental health conditions in a community setting.
* Work closely with multi-disciplinary teams to deliver person-centred care, ensuring the best outcomes for individuals.
* Support the development and implementation of care plans, delivering therapeutic interventions and promoting recovery.
* Contribute to risk assessments and care coordination in line with best practice guidelines.
* Collaborate with service users, families, and external agencies to enhance community integration and improve mental well-being.
* Participate in supervision, training, and development opportunities to further enhance professional growth and service delivery.

Person Specification
Knowledge & Skills
* An understanding of the NSF and Mental Health Act/ Mental Health Capacity Act requirements.
* Ability to be mobile across a geographical area to meet the demands of the role.
* Understanding of recovery principles in Mental Health.
Qualifications
* An appropriate up-to-date professional mental health qualification eg RMN, OT, or social worker on the appropriate professional body register and hold current registration with no restrictions. It will be an essential requirement for newly qualified social workers to demonstrate that they have completed The Assessed and Supported Year in Employment (ASYE) before commencing in post or there will be a requirement to complete this within the first 12 months of employment with the Trust.
* Evidence of relevant continuing professional development.
* Post registration training in relevant field.
Experience
* Postgraduate experience in at least two inpatient settings or substantial experience working within a mental health and/or learning disabilities setting.
* Managing a caseload of clients with a range of mental health problems.
