Job Summary:
Responsible for providing direct, patient-centered care and support to patients under the supervision of registered nurses or other healthcare professionals by performing essential duties related to patient care, including personal hygiene assistance, vital signs monitoring, and clinical support, ensuring patients' comfort, safety, and well-being.
Essential Functions:
1. Provide direct care to patients, including activities of daily living, vital signs monitoring, and personal hygiene support, ensuring comfort and safety.
2. Accurately record patient information, including observations, care provided, and changes in condition, in accordance with healthcare facility guidelines.
3. Foster a supportive environment by communicating effectively with patients and their families, addressing concerns, and providing information about care plans.
4. Work closely with the healthcare team to ensure coordinated and efficient patient care, participating in care planning and reporting any patient concerns or changes.
5. Assist with helping patients turn and reposition themselves in bed as needed and help them move between the bed and a wheelchair if they use one.
6. Listen to any health concerns patients may have and report those concerns to the nurses.
Qualifications:
Education: High School Diploma or Equivalent preferred. Can this role accept experience in lieu of a degree? No.
Experience: Experience in a healthcare setting 1-2 years preferred.
Knowledge, Skills and Abilities:
1. Strong interpersonal skills.
2. Familiarity with patient care procedures, safety protocols, and infection control practices.
3. Strong communication skills.
4. Ability to work effectively as part of a team.
