Job summary The Care Home Liaison Team is a borough wide multi-professional team, including psychiatry, mental health nurses, clinical psychology, occupational therapy, and support staff. The team provides assessment and interventions to support those with a mental health problem (including dementia) living in care homes. You will work as a member of the multi-professional team providing high quality healthcare support, assisting in the assessment, planning, implementation and evaluation of programmes of care as designated by the multi-professional team. You will provide this care under the direction of the qualified team members, across Care Home's in Rotherham. You will be required to work on your own for periods of time without direct supervision. You will be required to travel independently to a variety of localities across Rotherham, therefore access to transport is essential. All care and activity will be undertaken in accordance with the Trust's objectives and values. Main duties of the job To be responsible for an allocated number of individual service users, assisting and contributing to the planning, implementation and review of care plans. You will provide this care under the direction of Lead professionals, across Care Home's in Rotherham. To be an integral part of the Distressed Behaviour Pathway, gathering information and feeding back to lead professionals in a timely manner. When appropriately trained, complete a range of physical health checks as requested by Lead Professionals including physical observations, blood tests and ECG's. To be able to participate along with service users in a range of meaningful therapeutic activities following clinical specialist assessment such as supportive advice, education and support health promotion. To support service users to understand and manage their mental health needs in line with their care plan. To be able to engage effectively with service users and their carers, demonstrating relationship building. To communicate with external agencies on clinical matters as appropriate within the general scope of the post. Attend/participate in meetings/case conferences as required in relation to allocated service users.
