We wish to recruit nurses to work on the bank across Brushwood and Millvina care homes.
We are looking for applicants who have a clear understanding of developing the role of a nurse who can assess patients within a community.
We work predominantly but not exclusively with older people. Our focus is ensuring patients' health needs, often arising from multiple co-morbidities and frailty, are met with the appropriate level of care. We work alongside the MDT to gain the best patient outcomes and facilitate patients to their preferred place of care, avoiding delay where possible.
Candidates should demonstrate the ability to work in a team that delivers innovative, responsive services to meet both the needs of patients and the organisation.
We are committed to staff development, and you will have a named supervisor, the support of experienced colleagues, and the opportunity to attend courses and training, including leadership development.
The post holder will autonomously complete health needs assessments and continuing health care needs assessments to ensure that the appropriate care is in place. The post holder will ensure excellent levels of customer service for all patients/relatives and staff and will demonstrate improvements in quality and patient experience.
Working hours are 08:00-20:00 and 20:00-08:00. To fill the shifts, there are shorter shifts comprising of 07:30-15:00, 13:30-20:00, and 19:30-00:00.

To work in collaboration with operational partners across the health and social care system to develop, monitor, and expedite patient management plans for patients with complex needs. To facilitate, enable, and support interdisciplinary decision making in order to achieve optimal and safe patient flow and the utilisation of all available resources to support the patients. The post holder will provide focused support to patients, carers, and staff covering all aspects of the admission and discharge process for patients admitted.
