Ortus Health is seeking a dedicated Remote Care Coordinator to ensure patients receive comprehensive and coordinated care throughout their healthcare journey. This fully remote, full-time role involves working closely with patients and healthcare providers to facilitate communication, manage appointments, and provide support to enhance the overall patient experience. Responsibilities
* Serve as the primary remote point of contact for patients, addressing inquiries and providing information regarding services and care options.
* Coordinate and schedule virtual patient appointments, ensuring timely follow-ups.
* Maintain accurate electronic patient records and documentation in compliance with healthcare regulations.
* Collaborate with healthcare professionals to develop and implement individualised care plans.
* Monitor patient progress and communicate updates to relevant stakeholders.
* Assist patients in navigating the healthcare system, including understanding processes related to referrals and available community resources.
* Provide administrative support in a virtual environment, including managing correspondence and maintaining digital records.
Skills & Experience
* Strong organisational skills with the ability to manage multiple tasks efficiently in a remote setting.
* Excellent communication skills, both verbal and written, to effectively interact with patients and healthcare professionals.
* Highly skilled with computers, including proficiency in office software applications and healthcare systems for documentation and scheduling purposes.
* Empathetic approach towards patient care with a focus on enhancing the patient experience.
* Ability to work collaboratively within a remote team while also being self-motivated and proactive.
* Prior experience in a GP practice is essential.

£23,900 per annum based full time hours increasing to £25,000 once training is passed.
