Nightingale Valley practice are an active training practice covering 14,900 patients.
We are a very friendly and supportive team, co-located in a purpose-built Health Centre with a wide range of associated health services. Our team has a strong multi-disciplinary working ethos and enjoy supporting each other. Working closely with GP colleagues, the post-holder will deliver several Treatment Room clinics providing assessment; treatment; screening; health education services and advice to patients.
We are looking for the post-holder to work 30-37.5 hours per week.
Main duties of the job
You will ideally have knowledge and experience of dealing with chronic disease reviews, in addition to other Practice Nurse tasks. We would welcome applications from candidates who have experience working with QOF/IIF targets and in supporting, mentoring and developing team members.
You will need to be NMC registered with a minimum of 1 year experience working in primary care. The ideal person will be empathetic, enthusiastic and highly motivated and confident working alone or as part of a team.
The postholder would be responsible for contributing towards our annual campaigns, including planning and coordinating clinics, and overseeing QOF domains to ensure we are meeting required targets for chronic diseases. In addition, you will be expected to take part in the extended hours rota for the practice which can include some early/evening shifts and occasional weekend working.

Job responsibilities
Your responsibilities will be:
General Clinical
* To provide high quality patient care.
* Participate in setting standards for care and auditing clinical outcomes.
* Promote & participate in maintaining a safe, comfortable & therapeutic environment.
* Participate in developing strategies and implementing health promotion programmes for clients and carers and facilitate screening clinics.
* Take responsibility for the safe storage and administration of drugs and equipment.
* Provide information, advice and support to patients face to face and via the telephone.
* Involved in team discussions to ensure best practice.
* Chronic Disease management (Diabetes, Asthma, CKD, COPD, Hypertension, Heart failure, Ischaemic Heart Disease).
* Cervical Smears.
* Leading the treatment room team.
* Overseeing training and development for the treatment room team.
* To assist in decision making involving the treatment room team.
Treatment Room Clinical Skills required
* Venepuncture.
* Urine testing.
* Swabs.
* ECG procedure.
* Assistance in Minor Surgery.
* Infection control management of equipment.
* Immunisation and Vaccine administration.
* Cervical Cytology.
* Health promotion.
Organisational
* To maintain accurate records and data on Treatment Room activities, using the EMIS computer programmes.
* To work as an effective team member and promote a multi-agency collaborative approach to health care.
