Key Duties - Medicines Safety Collect, input and analyse medicines incident data to identify themes and trends and prepare reports including recommendations. Support the reporting of near misses medicines-related incidents across the organisation. Contribute to the review of pharmacy medicines safety incidents. Utilise tools and techniques to ensure systems review is undertaken andinvestigation and response are in line with the Patient Safety IncidentResponse Framework. Support the development and delivery of systems for sharing lessonsand learning from incidents. Support the medicines safety leads in recording, monitoring and feeding back on medicines risks. Undertake investigations involving CD balance discrepancies ensuringmisappropriation documentation and escalation pathways are followed. Support the proactive monitoring of CD use across the Trust utilisingdiversion dashboards and investigating any areas of concern highlighted. Assist with the design, co-ordination and completion of medicine safety, medicines management and CD audits, the input and analysis of data and the production and implementation of action plans. Provide supervision and support to staff undertaking the CD audits and assist with CD discrepancies they identify. Assist in writing and reviewing relevant departmental and organisationalpolicies and procedures. Support actions in response to NHS England alerts and notices andensure that responses are provided in a timely manner. Assist with updates to Trust medicines management training content. Interact with a range of colleagues to fulfil the responsibilities of the post including healthcare professionals across the multidisciplinary team.
