An exciting opportunity has arisen for a highly motivated and enthusiastic Nurse to join the Memory Assessment Service. This service covers the South Leeds area. The role involves the assessment and post-diagnostic support of people with or suspected Dementia.
You will work closely with a multidisciplinary team and be able to follow an agreed intervention plan with their support and guidance.
You will be encouraged to undertake professional development and be supported in participating in the PDP process, clinical and management supervision. As an active member of the team, you will also be involved in service improvement initiatives within the wider Care Group.
Main duties of the job
The Leeds Memory Services is a city-wide service with a focus on assessment and diagnosis of dementia, serving to meet the needs of the people of Leeds who are experiencing memory problems.
You must be able to demonstrate a passion for working with people with Dementia, be committed and motivated to working with older people, and take an active role in service development.
The post holder must be able to act as a role model for others and demonstrate a high level of clinical judgment, decision-making, and communication skills.

* Degree or Diploma in Mental Health Nursing

* Car driver essential with access to a car
Experience
* Working in a qualified role in a Mental Health setting for at least 6 months
Knowledge
* Demonstrate a desire to work with older people and have an understanding of working with people with dementia

Salary: £29,970 to £36,483 a year Per Annum Pro Rata
