Main duties of the job
As a qualified Physical Health Nurse, you will lead and coordinate all aspects of the physical health needs of service users, while also supporting your colleagues and promoting good teamwork with the multidisciplinary team, such as GPs and other physical health departments. You will be responsible for monitoring observations and assessments to identify any actions that need to be addressed and documented. The role requires adherence to the NMC Code of Conduct, developing and supporting physical health interventions, and engaging in practice development, evaluation, and audit activities.

* Lead and coordinate all aspects of the physical health needs of service users.
* Support your colleagues and promote good teamwork with the multidisciplinary team.
* Monitor observations and assessments to identify any actions that need to be addressed and documented.
* Adhere to the NMC Code of Conduct and facilitate compliance with local, Trust, and national standards of practice.
* Develop and support physical health interventions using knowledge of current best evidence-based practice.
* Engage actively in practice development, evaluation, and audit activities relevant to physical healthcare and/or service areas.
* Work collaboratively with primary health, developing and maintaining links with GP practices and other physical health departments.
* Monitor and maintain Physical Health Dashboards.

Where you will be working:
Ty Glyn Ebwy has recently been acquired by Elysium Healthcare and has started a transformational journey. The site will initially be an 8-bedded unit with capacity for 15 beds and is a specialist eating disorders service for female adults.
