An exciting opportunity has arisen for a senior band 6 Occupational therapist to join an established team working with a mixed caseload of community rehabilitation, return home from temporary placement, moving and handling assessment and palliative care. The team is based in North East Lincolnshire, assessing individuals in their own homes or care homes locally. Where the successful applicant does not meet all essential criteria this post may be offered as band 5-6 developmental opportunity with clear objectives.
Main Duties of the Job
* Undertake comprehensive assessment of patients including those with diverse or complex presentations or multiple pathologies; use advanced clinical reasoning skills, including the assessment of cognitive and perceptual impairment, interpreting these assessments and providing appropriate recommendations, considering appropriateness of a DFG recommendation or equipment or additional rehabilitation where appropriate.
* Assess and manage clinical risk within own and delegated patients.
* Assess patient understanding of proposals; gain valid informed consent and work within a legal framework with patients who lack capacity to consent.
* Represent the Therapy service and/or individual patients at MDT meetings to ensure the delivery of a co-ordinated multidisciplinary service.
* Work as a lone practitioner with telephone support from a senior Occupational Therapist if required.
* Be responsible for maintaining accurate, comprehensive patient treatment records in line with COT Standards of Practice and comply with COT Code of Ethics and Professional Conduct and national and local policies and procedures.
* Be aware of Trust policies and statutory requirements such as Health and Safety, Equal Opportunities, COSHH, Fire Safety, Complaints and Incident Reporting.
