This exciting role will promote the uptake of annual health checks for patients registered with Serious Mental Illness (SMI) across all our 19 GP Practices in Calderdale. Building on existing efforts, this role will also support practices in Calderdale to deliver the highest quality SMI Health Checks for their patients.
Working closely with our GP Practices, Mental Health Practitioner roles, Personalised Care Teams, VCSE, and patients registered with an SMI, this role will promote personalised care and address health inequalities.
Main duties of the job
Promote both the quantity and quality of SMI Annual Health Checks by working with agencies including primary care, secondary care, local authority, and VCSE teams across Calderdale to build on and embed existing work in this field.
Identify and address health inequalities of those with an SMI accessing general practice.

Job responsibilities
Support practices to implement the lessons from SMI annual health check pilot work.
Continue interoperability and bridge the primary-secondary care gap.
Build SMI multi-disciplinary teams within practices and across practices, bringing in other mental health transformation roles.
Encourage expert patient coproduction, learning from lived experience.
Work to encourage the uptake of individuals that have not attended annual health checks, including the identification of barriers and working with partner agencies to address them, including connections with prisons and housing.
Work to close the loop of a health check, ensuring checks lead to support and interventions.
Represent PGPA at Emotional Health & Wellbeing Alliance meetings.
Review national Quality Outcomes Framework to support practices in understanding and implementing any changes relating to SMI.
Support the implementation of national guidance including improving physical health of individuals with SMI.
Present lessons learned and best practices at learning events.
Provide regular updates to stakeholders within the steering group.
Prepare monthly highlight reports using PGPA template.
Person Specification
Experience
* Experience working in a mental health/social care or related VCSE setting.
* Understanding of the complexities of health conditions and barriers to accessing services for individuals with complex mental health needs/SMI.
* Experience of partnership/collaborative working and building relationships across various organisations.
* Understanding of health inequalities and measures to combat them.
* Aware of managing strategies for change within a workplace setting.
* Experience in the use of digital/clinical systems such as EMIS or System One.
* Aware of the wider network of mental health provision across Calderdale, including VCSE.
* Experience of coproduction with stakeholders.
Skills/Ability
* Basic IT skills such as Microsoft Word, PowerPoint, and Excel.
* The ability to prepare and deliver training.
* Excellent interpersonal skills.
* Experience in multi-agency working.
* Key role in collating and presenting relevant information in meetings.
Other requirements
* Flexibility to work outside core office hours.
* Maintain confidentiality at all times.
* Full UK driving licence.

* Educated to degree level and/or training around mental health.
* Experience of working with people with complex mental health needs.
