This is an ideal opportunity for current band 7 nurse to continue their development into Parkinson's Disease and Movement Disorders, or for a band 6 nursing wanting to expand their knowledge in this area. The post holder will act as a point of contact for patients experiencing problems or deterioration, providing highly specialist knowledge, skills and advice to clients and a source of expertise to health and social care professionals and others as necessary. Main duties of the job The post holder is responsible for the assessment of care needs, implementation and evaluation of programmes of care and the setting of standards of care with indirect supervision. You will provide nursing expertise to a specific patient group and will support clinical colleagues in providing a service to the PD client group. As a member of the Elderly Care team you will develop professional relationships with CoTE Consultants, Community Geriatricians and all relevant multi-disciplinary teams to enhance the service and care pathways, supporting their families and carers. This includes linking closely to Parkinson's UK staff locally. You will assess individual patient needs for those living with Parkinson's to improve their quality of life by identifying, co-ordinating and linking to appropriate care services and therapies throughout the care pathway. You will work independently to support patients and their carers to maximise on the benefits of the Parkinson's Nurse Specialist role on care programmes, developing patient focused goals, encouraging self-care and self-management approaches. The post holder will support the development of innovative evidence practice and be responsible for audit and monitoring the quality of the Parkinson's service being provided.
