This exciting new role has been developed to support the new Hospital at Home (H@H) and Urgent Community Response (UCR) multidisciplinary team working across Hambleton and Richmondshire.
This is an opportunity for those who are interested in working in the local community and being part of an exciting new model of working in the community localities of Hambleton and Richmondshire, working alongside nursing and therapy to help bridge the gap between home and hospital.
The UCR Team will work with patients that can be assessed in their own home to prevent hospital admission and to facilitate early discharge. You will provide both rehab and nursing care activities following a person-centred plan within the patients' own home.
The role will require some flexibility and this will require the post holder to work a mixture of day and night shifts across Hospital @ Home and UCR in response to service demands and therefore you would need to have access to a vehicle and be able to travel throughout the area.
Applicants must have Numeracy and Literacy Level 2 (GCSE 6-9)
Main duties of the job
To work as part of the Integrated Community Urgent Community Response (UCR) therapy/nursing team to deliver a 7 day, 24 hour same day UCR service based in the community, to support discharge to assess and admission avoidance. To work flexibly to meet the changing needs of the patients.
The Clinical / Integrated Support Worker Higher Level is a member of the team who will support patients at home during a 72 hour period of crisis until a care package can be instigated.
The post holder will undertake a broad range of care activities, including personal care without direct supervision of a Registered Practitioner but will feed back findings.
Each person that you work with will have individual needs, but the kind of support you might give includes:
1. Assistance with washing and dressing, getting in/out of bed on/off the toilet.
2. Prompting with medication
3. Base line observations
4. Fitting of equipment
5. Venepuncture
6. Bladder Scanning
7. Simple Dressings

Essential
1. Numeracy and Literacy Level 2
2. Care certificate
3. Level 3 vocational qualification or Short courses and completion of competency workbook equivalent to Level 3 in a range of patient and clinical care duties and/or Equivalent knowledge and experience in a range of clinical care duties equivalent to Level 3; - Theoretical knowledge - Experience of a range of care and related procedures - clinical observations - knowledge of relevant legislation
Knowledge and skills
Essential
1. Analytical and judgement skills to make decisions based on patient / service user interactions and observations and metrics.
2. Basic IT skills to use electronic patient records and other Trust systems.
3. Understanding of the importance of robust, accurate, clear, timely documentation.
4. Working knowledge of legislation underpinning care provision.
5. Timekeeping, planning and prioritisation skills.
Experience
Essential
1. Experience of working in health or social care.
2. Experience of undertaking and recording clinical observations and escalation of changes or findings.
