Job summary We are looking for enthusiastic and motivated pharmacists to join our friendly, supportive, pharmacy workforce at Wrexham Maelor Hospital. Within the Integrated Health Community East, we value our integrated pharmacy team who are all essential in providing a high quality pharmacy service to our patients. The post provides the opportunity to rotate through several clinical areas whilst undertaking the HEIW Post-registration Foundation training programme with Cardiff University. There are a wide range of rotations available including surgery, medical, specialist clinics, prison, paediatrics, emergency department, medicines information, critical care, aseptics, primary care, mental health and community services. Within each area you will have the ability to learn from and be supported by a specialist pharmacist. This provides excellent career development, with further learning opportunities via professional courses/qualifications, as the department has close links with Cardiff, Bangor and Wrexham Universities. Training is supported by the Foundation Framework for Pharmacists, which provides an ideal platform for professional and personal development, and is recognised at a national level. The pharmacy team at Wrexham Maelor Hospital is well-established within the wider multi-disciplinary team, allowing collaborative working. This allows you to have direct involvement in patient care and influence treatment choices by being involved in decision-making at the point of prescribing. Main duties of the job Provide a clinical pharmacy service to an allocated clinical area in accordance with professional standards and local policies. This will include: Take patient drug histories Review medicines to ensure safe and effective use; including review of prescriptions for completeness, appropriateness and legality, To communicate recommendations on drug therapy to doctors, nurses and other relevant healthcare professionals as appropriate Responsible for medicines supply To ensure the smooth, swift and safe transfer of patients care, liaising with carers, nursing and residential homes where appropriate and providing clear information on discharge medication (whether verbal or written) to patients or their carers. Formulate patient specific pharmaceutical care plans, taking into account results of investigations, response to treatment, and adverse effects Advise on therapeutic drug level monitoring for specific drugs Inform and educate patients about their medicines Provide pharmaceutical information (therapeutic, legal, procedural) to medical, nursing staff, other healthcare professionals and general practitioners where appropriate Report suspected adverse drug reactions to the Committee on Safety of Medicines using the yellow card reporting scheme The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
