Job summary Crays Primary Care Network are looking for an enthusiastic, forward-thinking Clinical Pharmacist to join their ever-growing team. Hours 0.4 to 0.6 WTE The Crays Collaborative PCN is a network of 6 GP surgeries which are Broomwood Surgery, Crescent Surgery, Derry Downs Surgery, Gillmans Road Surgery, Poverest Medical Centre and St Mary Cray Practice. Together they cover a patient population of over 35,500 patients, most of whom live in deprivation. Two of their practices are training practices. They are a highly supportive PCN whose practices work closely together, understanding the importance of a good work life balance with an open door ethos. The PCN caters to the healthcare needs of their patients by ensuring they make the best use of collective resources across practices, local healthcare providers and professional groups to allow greater resilience to primary care. To achieve this, they are supported by additional roles such as paramedics, Clinical Pharmacists, social prescribers, Health and wellbeing coach, nurses, care coordinators, mental health practitioners. You can expect to work across no more than 2 to 3 practices across the PCN. Occasionally group activities will take place at the wellbeing cafe. Main duties of the job Improve the physical mobility of patients who suffer from illness, injury, disability or aging Provides diagnoses, determines effective treatments and educates patients Evaluates patients physical dysfunction, develops personalised care plans and administers therapeutic treatments. Engages directly with patients assessing their physical conditions, diagnosing disorders and proposing treatment plans.
