Support Worker Salary Starts at £25,115, rises to £26,425 after 12 months and to £27,355 after 24 months (pro rata) This is a 7 day service and weekend working is required on a rota basis Support Worker Job Profile The Role You will be based at our Sheffield Office and will support service users on the mental health framework within shared living environments as well as delivering outreach support to clients in their own tenancies. You will deliver high quality support to clients in order to achieve the aims and objectives as set out in their recovery focused, person centred support plan. You will support clients in ways which promote both their wellbeing and independence. The role will involve supporting clients with daily living skills including shopping, attending their appointments, accessing community resources, health care and accessing and dealing with benefits to maintain a safe and comfortable living environment. Responsibilities include: Deliver 1:1 support to clients in line with the goals set out in their support plan Keep up to date, accurate and professional recordings of support delivery Liaise with external agencies and professionals involved in clients support Work flexibly as part of the staff rota to ensure safe staffing levels are met Encourage and support service users to take part in activities Encourage service users to contribute fully to their care and support plan Support service users with personal administration, form filling and making benefit claims as necessary This is not an exhaustive list of duties. Qualifications / Requirements: Experience working with individuals with mental health issues in a paid or voluntary capacity. A valid UK driving license and access to own vehicle. Business insurance will be required. An understanding of relevant legislation relating to vulnerable adults Experience of working in partnership with Local Authorities and Health/Social Care professionals NVQ level 2 in Health and Social Care is preferred Intermediate IT skills which must include Outlook, Word and Excel Effective and clear written communication skills in report writing, risk assessments and form completion Excellent time-management skills Specific tasks To work in line with person centered SDS support plans to ensure tasks and objectives identified are achieved within agreed time scales. To build the skills and confidence of service users to empower them in maximising their independence To work in partnership with other agencies to maximise service delivery Support service users to maintain a safe, clean and comfortable living environment within their accommodation Offer advice and support to negotiated and agreed support planning to enable them to develop their skills for independent living. Assist service users to maximise their income and to manage their personal budgets. Support service users to understand their rights and responsibilities under their occupancy agreements. Support and motivation to service users encouraging them to achieve personal goals. Carry out regular service user risk assessment and ensure it is properly recorded and copied to relevant officers in the organisation. Monitor occupancy to ensure that properties have not been abandoned or damaged and take prompt action when it happens to avoid unnecessary costs to TH. Where appropriate, help service users with budgeting. Negotiate with other bodies on behalf of the service user on the repayment of arrears such as rent owed to the Local Authority Housing Department. Assist in the delivery and development of service user activities. Keep all prescribed records in order that the value of the tenant support service can be audited and comply with confidentiality policy. Build links with external organisations, to maintain constructive working relations and to ensure service user’s needs are met. Provide cover for absent colleagues as directed. General tasks Attend and participate in training sessions or activities as required. Participate in team meetings and supervision as required, Perform other such duties as may be required to further aims of Target Housing within the Health and Safety requirements.
