Job summary Within a multi-disciplinary team assist in the co-ordination and delivery and development of a comprehensive cardiac rehabilitation and secondary/primary prevention programme as part of Wirral Community Cardiology Services across three main sites in Wirral. Main duties of the job The role will include managing a caseload of patients from referral though to discharge as well as the assessment of functional capacity and the prescription of exercise for a wide range of complexities. The role also involves working within the Tier 3 weight management service, delivering exercise guidance and assessment for those accessing the bariatric pathway.
