We are looking to recruit a highly motivated, forward thinking, committed and enthusiastic Nursing Associate with excellent communication skills to join our Tissue Viability Team. You will be pivotal in helping us to develop this new and exciting role within our team and act as a role model to current and future trainee nursing associates.
You should have excellent clinical, written and verbal communication skills and have completed the Trainee Nursing Associates course, gaining NMC registration.

Main duties of the job
The nursing associate role has a breadth of knowledge across the lifespan of patients and across the fields of nursing; providing holistic and person centred care and support for people of all ages and in a variety of settings. The nursing associate works independently under the leadership of registered nurses who work within the sphere of nursing and care.
Nursing associates are equipped with the knowledge, understanding, skills, attitudes and behaviours relevant to employment as a nursing associate and will work to a nationally recognised code of conduct.
Job responsibilities
Main Duties and Responsibilities
1. Provide and monitor care in the community setting:
* Provide compassionate, safe and effective care and support to people in a range of care settings.
* Monitor the condition and health needs of people within their care on a continual basis in partnership with people, families, and carers.
* Contribute to ongoing assessment recognising when it is necessary to refer to others for reassessment.
* Communicate effectively with colleagues, providing clear verbal, digital or written information and instructions when sharing information, delegating or handing over responsibility for care.
* Recognise and report any situations, behaviours or errors that could result in poor care outcomes.
* Undertake lower limb re-assessments and wound management using the appropriate documentation following the full training and assessment of own competence levels by a regulated professional.
* Record and analyse clinical tasks using existing clinical knowledge and experience; Measure and record blood pressure, temperature, height & weight body mass index, Purpose-T assessments, nutritional assessment using Must tool.
1. Planning:
* Work alongside senior health professionals in planning the clinics and visits, prioritising according to agreed protocols and seeking advice when required.
* Participate in coordinating patient visits and planning reviews accordingly.

3. Promoting Health and Preventing Ill-Health:
* Support people to improve their mental and physical health and wellbeing.
* Engage in public health, community development, and in the reduction of health inequalities.
4. Communication:
* Liaise as appropriate with members of the Adult community teams.
* Develop good working relationships and liaise as appropriate with parents/carers, and other health, social or voluntary agencies.
* Communicate effectively with clients in complex situations.
* Engage in the communication of complex information regarding clients needs and any other useful information to the Adult Nursing Team and/or other appropriate medical services.
5. Budgetary Responsibility:
* Take responsibility to minimise incurred expenditure.
* Ordering and receipt of stock.
6. People Management:
* Actively participate in annual performance reviews providing evidence of attaining the relevant knowledge and skills required for the job.
* Attend mandatory and relevant training courses and maintain up to date clinical knowledge and skills.
* Participate in staff and service development through attendance at service meetings and in service training.
7. Clinical and Practice Governance:
* Observe and maintain strict confidentiality with regards to any information in line with the requirements of the Data Protection Act.
* Adhere to the Trust risk assessment and risk management processes.
* Participate in relevant emergency preparedness process for their team.
Person Specification
Qualifications and Training
* NMC Registered Nursing Associate
* Educated to a minimum level 5 Foundation degree level specific to nursing associate practice
* GCSE Grade A-C English and Maths or Level 2 in Maths and English
* Completion of End Point Assessment for applicants who have undertaken an apprenticeship route
* Care Certificate or willingness to train
* Related health and social care qualification (Level 3)
Experience
* Experience of working in a team as a registered nursing associate
* Experience of patient-centred care
* Experience of health coaching and promoting independent living
* Previous experience of working within the community setting
* Evidence of involvement in support/development of less experienced staff
* Experience in community or hospital based healthcare.
Knowledge and Skills
* Ability to communicate with members of the public and health and social care providers
* Ability to manage own workload
* Ability to take part in reflective practice and clinical supervision activities
* Understanding of basic physiology, e.g. normal vital signs, nutritional requirements, referral processes etc.
* Ability to work on own initiative
* Standard keyboard skills and ability to communicate through IT using packages such as Word, Outlook, Excel and PowerPoint.
* Report writing skills (referrals, Datix).
* Ability to work flexibly as part of a team.
* Time Management and prioritisation Skills and Willingness to learn appropriate clinical competencies.
* Understanding of evidence based practice.
