Job summary We are looking to recruit a energetic, driven person into the newly designed post of Patient Transport Coordinator. You will be support the transport manager for the day to day Non-Emergency Patient Transport Service (NEPTS. This critical service supports the organisation to provide safe transport for eligible patients. The Patient Transport Coordinator will ensure that the eligibility criteria for Patient Transport Services (NEPTS) are enforced and effective booking processes are in place. You will work closely with Ward Managers, Transfer of Care team and the Site Management Team to ensure a high quality discharge plan which delivers on the operational key performance indicators for the service Main duties of the job You will support our patients to have safe transportation home through our digital and telephony booking system, working closely with ward staff. Job description Job responsibilities Work closely with the Ward Managers, Transfer of Care Team, Capacity Flow Manager and the Site Management Team to ensure the timely discharge of patients. Attend the daily site capacity meetings to identify delays in discharges due to transport issues and effectively provide solutions to address delays in the service. Ensure daily contact with all wards and departments to encourage early booking and to check on potential delays in discharges. Attend ward tracker white board meetings when the Trust is on OPEL 4 alert for capacity flows. Promptly escalate to the Site Managers and Capacity Flow Manager any issues and delays that cannot be addressed. Be the point of contact between the Site Managers, Discharge Facilitators, and the Transfer of Care Team with full and detailed understanding of the transport KPIs and service deliveries as well as the transport criteria. Ensure that appropriate authorisation is given to approve patients eligibility for NEPTS. Encourage ward staff to follow the eligibility criteria by ensuring that in the first instance patients who can make their own way to hospital or home are encouraged to do so. This will require frequent negotiation and persuasive skills to ensure understanding and compliance. Ensure that requests for transport are based on a patients medical need where possible and assessed on an individual basis. Ensure that a maximum of one patient escort is permitted in line with the eligibility criteria. Monitor bookings to ensure that they are booked with at least 24 hours advance notice and 48 hours notice for high dependency complex discharges to facilitate effective use of resources. Individually scrutinise any bookings made after 4pm the day before travel and identity trends and put in measures to address late bookings. Triage bookings to ensure the correct category of transport is booked and that all special requests are identified at the time of booking, Ensure effective early communication processes are in place to providers for notification of bookings and cancellation of journeys.
