North Yorkshire 0-19 Service have an exciting opportunity for you to join our 0-6 Team supporting with the delivery of the virtual element of our service. You will be based in a team dedicated to delivering virtual contacts.
The role of the Health Visitor has a large proportion of virtual working, providing universal contacts across the whole of the North Yorkshire footprint, whilst also having an element of in-person working within localities across North Yorkshire.
The Universal offer of the Healthy Child Programme in North Yorkshire is mainly delivered using a virtual platform which supports us to target in-person support. This role is key to delivery of the Healthy Child Programme, providing an opportunity for you to develop skills, new ways of working and innovation for delivery of the universal Healthy Child Programme.
Please note this post requires the successful candidate to work from base at certain times and may require in-person contact with children, young people and families for service delivery along with virtual delivery of care.
Main duties of the job
The successful candidate will work with parents, carers, children and partner agencies to promote the physical, emotional and social development of children.
Contributing to the delivery of the Healthy Child programme through delivery of mandated contacts and targeted interventions as a Specialist Community Public Health Nurse (SCPHN), working in conjunction with a team to co-ordinate, plan, implement and evaluate a programme of care for children, young people and families including infant feeding support, developing and planning of intervention to support children and families.
Supporting the completion of comprehensive health needs assessment of clients, families and community and facilitate the mobilisation of appropriate resources to meet identified needs, linking with locality based Health Visitor and skill mix team where in-person support is required.
The post holder will ensure inter-agency/ professional partnership working between services, statutory and voluntary organisations to facilitate integrated approaches in meeting the needs of children.

The 0-19 commissioned service within North Yorkshire has a blended approach to the provided contacts, with patients seen within their own homes, nursery settings and virtually using MS Bookings software.

Person Specification
Qualifications
* First level registered nurse/midwife
* Specialist practitioner.(Hons, B. Sc. Hons, Health Visiting/ Public Health Nursing or equivalent)
* Minimum 2 years Post Registration experience as a qualified nurse.
* Recognised student mentoring qualification.
* Evidence of continuous ongoing professional development.
* Working knowledge of the 0-19 North Yorkshire contract.
Experience
* Minimum 1 year post SCPHN qualification.
* Prioritisation of complex workload
* Knowledge in Identification of Levels of Intervention
* Evidence of participating on a service development change.
