Community Rotational Occupational Therapist

The Manchester community response services offer a seven-day service, which entails working some weekends, bank holidays, and shift work on a rota basis. The post involves working across community services on a rotational basis, delivering holistic assessments & interventions to patients in their own homes/place of residence. Independent travel within the local geographical area to deliver the role is essential.
The rotations will include:
1. Discharge to Assess - delivered over 7 days 08:00-20:00
2. Home pathway - delivered over 7 days 08:00-20:00
3. Hospital at home - delivered over 7 days 08:00-20:00
4. Crumpsall Vale Intermediate Care - delivered over 7 days shift patterns including earlies (07:30-15:30) and lates (11:00-19:00)
With the above in mind, you must have experience of working in the NHS and in community services. The successful candidate will be able to demonstrate an appropriate level of clinical skills in occupational therapy, will be experienced in administering person-centred assessments, as well as implementing and evaluating occupational therapy interventions in a fast-paced working environment. You must have a recognised Occupational Therapy qualification and current HCPC registration.
Main duties of the job
The individual will manage a clinical caseload of clients, including those with complex and highly complex needs, with support from the band 7 therapists for highly complex patients, and supervise the band 4/3 support staff with caseload management.
The post holder will work without direct supervision as a lone worker in the community assessing, planning, implementing, evaluating, and modifying individualised treatment programmes in a variety of care settings e.g., patients' homes, clinic settings, residential homes, and intermediate care settings for patients with low to highly complex needs.
The post holder will work as a team member collaborating with colleagues to ensure patient needs and goals are met using approaches that promote choice, participation, and quality of life.
The post holder will ensure patients are triaged and managed on appropriate and timely pathways of care by liaising closely and working in partnership with other health and social care professionals including GPs, Consultants, Specialist equipment agencies, Social Service partners, and Voluntary agencies, and support the timely discharge of patients from the acute setting.
The post holder will provide complex professional and clinical advice to clients, family members, carers, and health and social care professionals including GPs, therapy colleagues, and care managers.

Person Specification
Qualifications
* Honours degree in Occupational therapy / Occupational Therapy Diploma
* Evidence of continuing professional development
* Further health-related qualification
Experience
* Significant band 5 experience
* Evidence of experience with a broad range of general rehabilitation conditions
* Previous experience at band 6 level
* Previous Community experience
Skills and Knowledge
* Ability to manage complex caseloads
* Ability to work autonomously and as part of an MDT
* Well-developed communication skills with the ability to communicate and extract complex information
* Ability to produce clear concise written information.
* Ability to demonstrate a sound understanding of clinical governance and risk
* Knowledge of research and audit
* Experience of supervision of occupational therapists or rehabilitation assistants

* Hold a UK driving licence and have access to own vehicle
