An exciting opportunity has arisen for a band 6 physiotherapist to join our established Community Neuro Rehabilitation and Stroke Service. Care is currently delivered to patients in their own homes or usual place of residence between 8am-5pm, Monday to Friday, excluding bank holidays.
The team offers full or part time roles, including flexible working which aims to achieve the right work-life balance, maintaining the wellbeing of our workforce.
Main duties of the job
This role requires day-to-day delivery of patient care in our Community Neuro Rehab Team. You will need to:
1. Use specialist clinical reasoning, critical thinking, reflection and analysis to support assessments, treatment and management of patients with long term neurological disease and stroke survivors with complex presentations.
2. Work closely with the MDT, communicate effectively with clients/carers and their families, and supervise, teach and develop other staff as required.
3. Support our patients within the community to enable them to live their optimum life, being self-caring and independent with their health needs following a stroke or deterioration in their long term neurological condition.
4. Demonstrate excellent communication and interpersonal skills with the ability to be adaptable, reliable, resourceful and approachable, evidencing high levels of personal and clinical credibility.
Job responsibilities
We are looking for a part time Physiotherapist to join our Community Neuro Rehab and Stroke Team, to provide rehabilitation for patients in their own homes in the geographical location of Bassetlaw. We are looking for someone who is able to demonstrate motivation, drive commitment and passion for promoting health, well-being and independence for patients with long term neurological conditions/stroke in the community.
This post will allow the successful applicant the opportunity to work collaboratively within an integrated multi-disciplinary team, providing specialist interventions to patients referred to the Community Neuro & Stroke service, including early supported stroke rehabilitation. The multidisciplinary team includes Occupational Therapy, Speech and Language Therapy, Physiotherapy, Clinical Neuro-Psychology, Specialist Nurses, Assistant Practitioners and Rehabilitation support workers.
The service enables the accelerated discharge of stroke patients into their home environment, providing specialist rehabilitation in the community. You will have a key role in helping facilitate and support timely discharges from inpatient settings such as the acute hospital by working closely and providing in-reach joint working with our close partners.
You must have a:
* Recognised Physiotherapy Degree Qualification
* HCPC registration
As part of the community neuro service, the post-holder will be required to have a valid full UK driving license and have access to a vehicle for work purposes; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
The service runs from 8am 5pm during Monday to Friday, excluding bank holidays.
BASE: The Community Neuro and Stroke Service covers the geographical location of Bassetlaw. This position will be based at Retford Primary Care Centre.

Qualifications/ Professional
* Degree BSc in Physiotherapy
* Evidence of post graduate study in stroke/neuro rehabilitation
* Clinical Educator training and evidence of student supervision
Experience
* Experience working in Stroke/Neurological rehabilitation MDT or with stroke/neurological patients
* Wide range of experience working at junior level to give good broad knowledge base.
* Supervisory experience to junior staff/students
* Experience of audit
* Community (Stroke) Experience
* Teaching experience
* Experience working at senior level
* Experience of working in the NHS
Knowledge
* Evidence of management of clients with Stroke/ Neurological rehabilitation needs
* Evidence based practice
