An Integrated Neighbourhood Team (INT) is a new service being established for Wantage. We have an opportunity for a passionate and organised individual to join us as the INT Co-ordinator. This is a pilot service and therefore the vacancy is fixed term to 31 March 2025 which will be reviewed upon the success of the pilot. This is a great opportunity for someone with experience of working in the primary care environment to develop further skills and progress their career.
The INT Coordinator will be a central point of contact and navigator between their designated Primary Care Network (PCN) and the INT multi-disciplinary team (MDT) establishing effective communication processes across primary care, community services, mental health services and the voluntary/third sector.
The role of an INT Coordinator will be used to working in a multidisciplinary environment, with changing priorities and challenges. The INT Coordinator will work in collaboration with the INT multi-disciplinary team and will be responsible for liaising with key workers to manage patient information to support MDT meetings including capturing outcomes and evaluation.
Main duties of the job
The INT Co-ordinator will provide administration support to the INT clinical team but also provide support for GP Practices, patients, carers and professionals to improve patient care, access and navigate resources and services which will help those patient groups prioritised by the Clinical will workclosely with the GPs, community services, PCN Team, Local District Councils, Social Care and voluntary sector organisations.
Administration tasks include:
1. Utilise Population Health Intelligence to proactively identify target group patients to improve patient care and wellbeing in the locality.
2. Identify and agree key priorities for the local area with the Clinical Leads. Review performance of INT and benefit to patients
3. Ensure that the reduction of health inequalities is embedded into practices across the INT based on local population need, evidence of best practice and effectiveness.
4. Ensure clinical staff are available for clinics noting annual leave etc. Invite Patients / Book appointments (including cancelling and rearranging when necessary).Ensure patient records are updated.
5. Provide reception service for clinics signposting patients & making referrals as appropriate.
6. Resolve any queries / problems / complaints.
7. Stock Control: Order equipment / consumables and stock / restock clinical rooms.
8. Process invoices for payment
9. Arrange for equipment to be PAT tested / calibrated
About us
Church St and Newbury St Practices are a two practice Primary Care Network with a combined population of circa 34K. We share the Health Centre with a pharmacy, district nurses, health visitors and school health nurses.
Alongside our teams of GPs, Nurses and HCAs we have a multi-disciplinary team to include pharmacists, pharmacy technicians, care co-ordinators, social prescriber, first contact physiotherapist, children's and young persons well being workers, a mental health practitioner, frailty practitioners and nursing associates.
Both practices have an ongoing staff welfare programme and encourage all members of the team to come together for morning briefings, coffee mid morning and lunch time walks.
There is ample free parking on site but also good access to public transport. Wantage is a growing market town at the foot of the Ridgeway mid Oxford, Reading and Swindon.

This is an exciting opportunity to play an important part in developing this new service for the patients of Church St and Newbury St Practices who have long term conditions (LTC), haven't attended for their annual health check for a year or more and have perhaps had hospital admissions for the same LTC.

To act as meeting secretariat in the preparation of agendas and circulation of relevant papers/minutes to participants for MDTs and the INT committee. Attend to minute the meetings and ensure production of good quality minutes and dissemination of minutes within agreed timescales.
Be sensitive to the local political landscape and sensitively work across organisations, often against conflicting priorities, to bring consensus enabling effective reduction of inequalities.
Work collaboratively with the INT Clinical Lead(s), local health care providers and wider system partners to support development and design of INT, modelling strong collaborative behaviours that focus on improving patient outcomes and encouraging engagement.

10. Experience of administration procedures & diary management
