We will support with any development staff member needs and provide in-house training relevant to our area. We can access training from community teams.
Regular supervision and yearly appraisal are part of the role.
The role of the discharge Co-ordinator is to provide support to wards, attend board rounds as discharge starts on admission, and ensure that all patients are discharged safely with the care they require. You will be able to escalate any delays in care or discharge planning and liaise with other team members for patients from care homes and those approaching end of life.
