Lead Nurse Children and Young People's Continuing Care

There is an exciting opportunity for a Mental health and/or Learning disability nurse with experience of working with children and young people with Neurodiversity, mental health and/or a learning disability to join the Children and Young Peoples Continuing Care team in Leeds.
The Lead Nurse will have a key role in leading the multi-disciplinary assessments, case managing and commissioning packages of care for children and young people eligible for CYPCC funding. The Nurse assessor works in partnership with children, young people and their representatives, and other professionals including health, education, and social care. The Lead Nurse will also be supporting the expanding the team and contributing to the achievement of the key performance indicators for the ICB by acting as the first point of contact for children and young people whilst always maintaining patient confidentiality.
Main duties of the job
This role will be responsible for:
1. Completing assessments for Children and Young people with challenging behaviours and emotional needs to determine eligibility for CYPCC funding.
2. Coordinating and completing MDT assessments and reviews in accordance with the National Framework.
3. Providing evidence-based holistic assessments within the agreed timeframes.
4. Supporting children and young people with the offer of a personal health budget to meet the assessed needs and outcomes.
5. Case management for children and young people eligible for funding via CYPCC.
6. Record keeping as per Leeds continuing care policy.
7. Working in partnership with children, young people, and their representatives.
8. Building and maintaining relationships with Local Authorities, providers, and hospital trusts.
9. Contributing to the ICB's achievement of their statutory duties.
10. Supporting the transition of young people to adulthood.
11. Contributing to the ICBs statutory duties for SEND.

Job responsibilities
The post holder will assist the ICB in delivering its responsibilities with regard to the National Framework for Children and Young Peoples Continuing Care.
1. To assess Children and Young People with mental health needs and or learning disability for Continuing Care working closely with education and social care.
2. To determine eligibility for Children and Young Peoples Continuing care and carry out reviews of care packages.
3. To work with the team to coordinate personal health budgets and implement the Care pathway for Leeds Children with complex health needs.
4. To provide high-quality, integrated assessments for children and young people across Leeds that will determine eligibility for Childrens Continuing Care.
5. To use clinical expertise to analyse and challenge health information and assessments to ensure that an accurate assessment of childrens needs are documented.
6. To support implementation of the personal health budget process.
7. To communicate effectively with colleagues, partner organisations, service users and their representatives.
8. To maintain a rapid and effective response to requests for advice and assessments.
9. To work within the timescales outlined in the Childrens Continuing Care Policy.
10. To work with colleagues to implement the care pathway for discharge of Leeds children with complex needs.
11. To meet statutory duties under the SEND Code of Practice, engaging in the Education, Health, and Care planning process to ensure alignment of EHCP and Continuing Care.
12. You will be expected to demonstrate knowledge and understanding of all aspects of safeguarding children and work within organisational policies.
13. All staff are expected to work flexibly across all functions according to the skills and requirements for individual roles.
14. Support development of policies and protocols as appropriate.
15. Lead on a specific project or area of work as directed by the Manager, deliver outcomes with a managed approach, providing timely updates as required.
16. Lead and co-ordinate delegated projects to deliver agreed outcomes and report progress as necessary.
17. Day to day leadership and line management, support setting direction of the team ensuring all team members have clarity of purpose.
18. Produce timely function specific advanced reports, papers and analysis using prolonged concentration where required.

Qualifications
* Registered Mental Health Nurse (RMN)/ Learning Disability Nurse with experience of working with Children and Young people.
* Educated to degree level or equivalent in experience.
Experience
* Experience of working with Children and Young people with complex behavioural needs.
* Advanced assessment skills.
* Experience of Continuing Care.
