Salary: £54000 to £65250 a year plus enhancements attached to KSF and dependent upon experience
Full time, part time and bank positions available
Working patterns flexibility between 8am - 11pm Monday to Sunday

Main duties of the job
As an Advanced Clinical Practitioner working within the Oldham locality you will holistically assess and clinically examine patients with both minor and complex needs. You will have access to pathways and a broad Directory of Services (DoS) along with :
1. Direct access to a wide range of community-based services.
2. Referral rights to SDEC and secondary care.
3. Access to technology to support telephone and video consultations (full induction and training provided).
4. Facilities to see patients face to face (with support if you are expanding your scope of practice).
The service runs from two sites; the Urgent Care Hub in the Integrated Care Centre and our Pre Emergency Department (Pre ED) streaming service within The Royal Oldham Hospital.
The Pre ED streaming service includes working closely with The Royal Oldham Hospital to identify patients who present to ED with primary care presentations who can be streamed away from ED.
Youwill possess leadership qualities and provide support to the wider team, act asa positive role model and proactively identify areas for quality improvement.
You will have full access to our gtd learning academy and forums. This provides a great opportunity to grow and expand your clinician skills/knowledge and to excel as an ACP.
Using EMIS, you will consult with patients with undiagnosed/undifferentiated problems. You have access to the patients full clinical records and consult via telephone, video or face to face, according to patient needs.

ClinicalPractice
16. Assessand holistically clinically examine patients with both minor and complex needsthrough history taking, physical examination and appropriate use ofinvestigations to confirm diagnosis.
17. Plan,implement and evaluate an effective management plan for the patient, ensuringsafe discharge/referral and/or transfer to an appropriate health care service.
18. Provideinformation and advice on prescribed or over-the-counter medication onmedication regimens, side-effects and interactions.
19. Prioritisehealth problems and intervene appropriately to assist the patient in complex,urgent or emergency situations, including initiation of effective emergencycare.
20. Referand discharge safely ensuring that safety-netting advice is offered.
21. Triagetelephone referrals, calls from external clinicians, patients and others into arange of services appropriately as required.

22. Utiliseand demonstrate sensitive communication styles, to ensure patients are fullyinformed and consent to treatment.

Non-Medical Prescribing
27. If you are employed in a prescribingcapacity, you must only prescribe within your scope of practice and comply withthe gtd healthcare NMP governanceregistration processes.
28. You must maintain your knowledge and skillsand prescribe within your scope of clinical competencies.
29. The non-medicalprescriber is required to comply with organisational NMP governance processesand adhere to local and national prescribing guidance.

65. Registered health care professional, including first level nurse, paramedic, pharmacist or other AHP with MSc Advanced Clinical Practice or credentialing qualification or equivalent.
66. V300 Non-Medical Prescriber (gateway dependent) dependent on professional eligibility.
