We are looking for Experienced Advanced Clinical Practitioners to join our multi-disciplinary teams at Leeds Walk-in Centre (Burmantofts, LS9) and LGI (Leeds General Infirmary)
This is a new role due to the growth of the service and an enhanced model of care.
The Advanced Clinical Practitioner will assess, diagnose and plan care for patients; including prescribing of medications where clinically indicated. The ACP will manage acute presentations to the Walk-In Centre, as part of a team of autonomous Practitioners; in addition to using the clinical knowledge and skills to determine urgency and best pathway for patients presenting to the Walk-in Centre (streaming). Continuous professional development is key for our clinical workforce with the role having dedicated time for this and access to regular educational sessions. This is your opportunity to build an urgent care portfolio in a fast pace, dynamic service in Leeds.
The full-time equivalent for us is 40 hours per week. Part-time can also be considered and we welcome conversations about flexible working.
Our Walk-in Centre, located within the Inner Leeds area, has a diverse patient population who present with a variety of medical and social needs. The Walk-in Centre is open 7 days a week, 8am to 8pm including bank holidays. It is located on Cromwell Mount, Leeds, LS9 7TA.
Leeds General Infirmary, also known as the LGI, is a large teaching hospital based in the centre of Leeds, West Yorkshire, England, and is part of the Leeds Teaching Hospitals NHS Trust. You can find it on Great George St, Leeds LS1 3EX.
What you’ll work on
* Face to face consultations
* Undertake urgent primary care and refer elsewhere, when applicable
* Provide patient focussed, evidence-based practice to a high standard
* Supporting the wider MDT to ensure patients are seen by the right person
What we’re looking for
* First Level registered Nurse (NMC), HCPC professional/ Pharmacist
* MSc or equivalent in Advanced practice
* Advanced Physical Assessment and Consultation Skills Level 7
* Minor Illness Level 7
* A Non-medical prescribing qualification
* Minimum 3-year experience post level 7 qualifications in primary/urgent post qualification
* Paediatric Course Level 7 (desirable)
* Ability to see and treat patients with varying conditions
* Able to demonstrate excellent communication skills across all levels of the business
* Excellent organisational skills
