Virtual Ward Clinical Healthcare Support Worker
37.5 hours per week (Predominantly 08:00-18:00 (Other hours can be considered))

Main duties of the job
* To assist with pathways internally and externally which mainly involve travelling to locations outside of the Trust (Home Visits)
* To assist with the development of protocols and pathways within Virtual Ward
* To work within a Multi-Disciplinary Team to develop Virtual Ward within the trust.
* Within the remit of your role and under supervision, to contribute to the planning and completion of holistic assessments (not primary assessments) ensuring all care activities / assessments are clearly recorded and documented.
* To perform cannulation and Venepuncture as directed by nursing team.
* To perform vital signs monitoring, and Blood Glucose monitoring, reporting abnormal results to registered nursing staff as appropriate.
* To work within set guidelines and Trust policies recognising one's own abilities and limitations.
* To disseminate knowledge to other members of the Multi-Disciplinary Team.
* To liaise with Consultants/Senior Nurses and their Teams.
* To issue guidance to patients regarding ward pathways or protocols
* To Ensure all clinicians receive results promptly.
Education and Training
* Implement audits as delegated by clinicians to monitor.
* Participate in the Trusts appraisal system.
* Maintaining own professional and personal development and up to date knowledge of research in relation to this area of work together with relevant government policies and strategies
* Raise any training requirements.
Professional
* Ensure that the Ward Manager is informed of any professional concerns at the earliest opportunity,
* Always behave in a professional manner and adhere to the Trust policies.
* Adhere to the Trusts uniform policy.
Special Requirements
* The post holder will be expected to frequently exert moderate physical effort for short periods on a shift when undertaking clinical duties and should expect to be frequently exposed to unpleasant conditions, e.g. housing conditions, body fluids, e.g. faeces, vomit, blood, urine. when working clinically in the community and ward areas.
* The post holder will be frequently exposed to distressing / emotional situations during the course of a shift whilst delivering patient care to patient's homes, or ward areas.
* The post holder will be expected to be able to manage and prioritise work load when there will be competing demands for their time and attention.
* The post holder will be required to have access to a reliable vehicle, insured for business use, in order to facilitate travel for scheduled home visits.
Person specification
Experience and Knowledge
Essential criteria
* 18 months experience in acute care / hospital setting
* Understanding of basic physiology, e.g., normal vital signs, fluid balance nutritional requirements etc.
* Experience of working in more than one area/discipline within the clinical environment/setting
* Ability to work effectively as a team player under appropriate supervision, and as part of a multi-disciplinary team.
* Knowledge and experience of providing person centred care.
* Ability to escalate to relevant staff.
Desirable criteria
* Knowledge and understanding of the 6C's of nursing
* Experience as a lone worker.
Qualification and Professional Training
Essential criteria
* GCSE Maths Grades A-C, Levels 4-9, Functional Skills Level 2
* Evidence of CPD
Desirable criteria
* NVQ / Diploma Level 3 Clinical Healthcare / Health & Social Care / Level 3 Healthcare related qualification
