This is an exciting opportunity for a Band 6 Mental Health Nurse to join the Bexley Memory Service.
We are looking for a registered mental health nurse with experience in Memory services and/or Dementia care. The post holder will work as part of a well-established memory service, carrying out assessments, diagnostic appointments, follow-up appointments, group work, and carrying a caseload.
The post holder will need to be motivated, passionate about Dementia care, and be prepared to work as part of a team that is striving for excellency. They will need excellent communication skills, the ability to be flexible, work proactively, and work well as part of a team.
Main duties of the job
The post holder will carry out detailed memory assessments, provide clear presentations in MDT discussions, support diagnosis formulation, provide feedback to patients and families, carry out medication reviews, and support the post-diagnostic support offered within the service.

To ensure that own practice remains within legal, ethical, and professional parameters and is open to scrutiny from peers.
Must hold a current, valid first-level registration with the Nurses and Midwives Council. Management of caseload - clinical. Provide leadership, mentorship, and supervision for support workers, Band 5 nurses, and student nurses, as appropriate. Good computer literacy skills. Management responsibilities include the management of clinical caseload.

Clinical:
To accept care co-ordinator responsibility for a clinical caseload. To initiate assessments and treatment programmes in service users' homes and at the memory clinics. To facilitate the inclusion of social care services in the care plan in line with assessed needs, agreed eligibility criteria, and agreed procedures.
Respond to the needs of people in an honest, non-judgmental, and open manner, which respects the rights of individuals and groups. Actively engage with service users and their carers in the provision of holistic, needs-led care, which takes account of the physical, psychological, emotional, social, and spiritual needs of individuals.
Have an understanding of the needs and rights of carers and undertake Carers Assessments with a view to offering supportive services. To maintain accurate RIO records, including core and risk assessments, care plans, progress notes, and contacts as required.

Experience
* 2 years minimum experience as an RMN
* Experience of working with older people with mental health needs
