Registered Nurse/Social Worker/Registered Allied Health Professional? Interested in a new challenge using your excellent assessment skills? We are looking for qualified Senior Practitioners to join the Personalised Healthcare Commissioning (PHC) service to work with our Teams in supporting ICB`s in providing support for patients in receipt of health funding.The post holder will play a key role in supporting the Personalised Healthcare Commissioning service across the region. Working within the remit of the National Framework for Continuing Healthcare and other NHS funding streams, the post holder will deliver an effective and competent level of service and consistently deliver a client-focused service. You should be well versed in dealing with complex and sometimes contentious information. The Post holder should hold a full UK driving licence and have access to a vehicle to be able to fulfil the requirements of this post.If you are a confident, resilient, articulate and enthusiastic individual with a passion to constantly work to improve services for patients, are used to working in a challenging environment and like to find solutions/problem-solve, this could be the post for you.
Main duties of the job
The post holder will be expected to lead on Continuing healthcare assessments across the responsible integrated Care Board (ICB) area.
You will support and advise colleagues in relation to NHS Continuing Healthcare Funding ensuring current practice is being embedded into clinical practice, promoting best practice and compliance with the Department of health processes.
The role will be responsive to ICB priorities and expectations whilst working on their behalf and representing them through work with NHS Continuing Healthcare.

1. Healthcare Professional Registration, RGN, RMN, RNLD, AHP.
2. Extensive knowledge of specialist areas acquired through experience and training, plus further specialist knowledge of the continuing healthcare process and MDT working.
3. Negotiation and conflict management skills and the ability to influence.
4. Demonstrated capabilities to manage own workload effectively and make informed decisions in the absence of required information, working to tight and often changing timescales.
5. Effective communication skills.
