As a Clinical Pharmacist, you will work with our current Clinical Pharmacist Team to further develop and manage medicines optimisation services.
You will work in the Trent PCN practices, and therefore have the opportunity to build relationships with the primary care team.
You will work together to manage repeat prescription authorisation and reauthorisation, acute prescription requests, management of transfer of care, and systems for safer prescribing.
You will have the opportunity to develop areas of clinical interest in a patient-facing role, improve your prescribing skills, and enhance the management of patients with long-term conditions. Mentoring and support will be provided.
You will be supported through your CPPE pathway and your IP course if needed by our team of Pharmacists.
You will not be working in any practice alone.
Main duties of the job
You will provide primary support to general practice staff regarding prescription and medication queries. You will help support the repeat prescription system, deal with acute prescription requests, and conduct medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the GP practice(s).
You will provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.
You will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare, and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies, requiring motivation and passion to deliver excellent service within general practice.
You will work with the other Clinical Pharmacists in our large team, being fully supported throughout.
About us
K2 Healthcare is a GP Federation covering 16 practices: 10 in Grantham and surrounding villages and 6 in Sleaford. We cover 130,000+ patients across all sites. We have a team of 12 Clinical Pharmacists working for us, including 3 senior Clinical Pharmacists and 6 pharmacy technicians.
We are also working with Trent Care Network, covering five practices with a population of 40,000 to provide support and mentorship for their Clinical Pharmacists as they grow their team. This post will be working out of the 5 sites and will have the support of all the K2 Team.
Trent PCN has invested heavily in their clinical pharmacist programme.
The Clinical Pharmacist Team are all at different stages of completing their CPPE / IP course and are happy to speak to you regarding their experience with K2 Healthcare.
Job responsibilities
The post holder is a pharmacist who acts within their professional boundaries, supporting and working alongside a team of pharmacists in general practice. In this role, they will be supported by senior clinical pharmacists who will develop, manage, and mentor them.
The post holder will work as part of a multi-disciplinary team in a patient-facing role. They will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
The post holder will provide primary support to general practice staff regarding prescription and medication queries. * Two years post-qualification.
* Experience of working in Primary care - desirable but not essential.

* Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.

* May have completed CPPE Pathway.
