Job responsibilities: Assist the whole team to help deliver effective implementation of management of patients and their needs. Conduct patient assessments and take medical histories to provide comprehensive care. Assist in the management of chronic diseases by monitoring patient progress and providing education on self-management techniques. Educate patients on health promotion, disease prevention and healthy lifestyle. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care Advise patients on general health care and minor ailments with referral to GPs as necessary Maintaining efficient liaison with all areas of the primary care team and other agencies to assure continuity of care throughout. Clinical skills will include some or all the following (this list is not exhaustive): Women's health Family planning Cervical cytology Wound care Suture and clip removal Travel health Ear syringing Smoking cessation Long term conditions management: Asthma/Peak Flow Readings/Spirometry; COPD; Diabetes reviews (including food checks and care planning); Hypertension, Atrial Fibrillation, Heart Failure and CHD Management; CKD management; Undertake screening including BP checks, height, weight, BMI, Urinalysis, smoking status, etc. New Patient checks Over 75 years old Health Check Undertake venepuncture according to practice protocol Weight management Vaccinations and Immunisations Administer injections under an individualised prescription or PGD Continuously update your knowledge of the implications of immunisation and contra-indications Maintaining cold chain to ensure safe storage, rotation and disposal of vaccines and drugs. Requesting pathology tests, for example urine culture, swabs Assisting GPs with minor surgery. Infection prevention and control: Support patients and colleagues in adopting sound infection control measures Lead or participate in infection prevention and control internal audit. Apply infection control measures within the practice according to local and national guidelines. This includes: Universal hygiene precautions The collection and handling of laboratory specimens Segregation and disposal of waste materials Decontamination of instruments and clinical equipment Reporting and treatment of sharps injuries Dealing with blood and body fluid spillages Ensure the safe management and disposal of clinical waste and PPE Emergency management To be competent in anaphylaxis and resuscitation techniques. Support maintaining/ maintain emergency trolley/ emergency drugs. To attend updates when required. Other tasks will include: Maintaining and cleaning equipment used by the nurses and GPs Maintaining GP and Nurses rooms, stocking and rotating items as required Ensure the safe management and disposal of clinical waste and PPE Chaperoning and assisting patients who are being examined by another clinician. Maintain general tidiness and cleanliness of nurses and treatment rooms Supporting to order / ordering of Health Promotion Literature Supporting to order / ordering of stock from the District Hospital Ordering of vaccinations to maintain stock levels Participation in administrative systems in the Practice Maintain accurate patient medical records and documentation in accordance with legal and ethical standards. A duty to advise management of potential problems or errors within a range of assigned tasks Attend and participate in any Practice meetings when required. On occasion there may be a requirement to undertake home visits Any other delegated duties appropriate to the post In participation with the nursing and management teams, ensure the maintenance of equipment and stock levels relating to patient care Co-operate and participate as required in any research projects within the practice Participate in the education and training of students of all disciplines and the induction of all members of practice staff with special responsibility for nursing staff Liaison Maintain effective liaison with other agencies and staff concerned with patient care and with all other disciplines within the practice, with appropriate regard to confidentiality Professional development Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development
