We have exciting opportunities for Band 6 Nurses to work across our PCN Sites (listed below) within Surrey Downs Health & Care. If you are passionate about caring for people and making a difference within your community, we would love to hear from you!
The post holder is responsible with the continuing management of the patients and/or staff within their clinical setting, without supervision alongside their team lead and taking the chain of command in the absence of their team lead. This includes the assessment of care needs, the development, implementation and evaluation of programmes of care, the setting and maintaining of standards and the supervision and deployment of staff, if appropriate. The post holder will undertake facilitation of educational programmes for basic, post basic and in-service training, in close liaison with the Learning and Development Team.To work closely with the senior team member to facilitate patient focused, innovative research based practice, in line with Surrey Downs Health and Care objectives.
Main duties of the job
We are looking for nursing staff from all backgrounds including those more experienced, looking for a change, maybe wanting to work locally, excited by the difference of being out within the community with the benefits and challenges that brings.
Please note these roles require post holders to hold a current and valid deriving licence enabling them to drive in the UK and have access to a vehicle for work purposes.

PCN - Primary Care Network
It is a network formed from GP practice populations working in partnership with others to provide health and care to local communities.
What will PCNs do?
* Provide ongoing care and support
* Deploy the most appropriate staff member(s) in a coordinated and timely way to best use community resources
* Reduce risk of admission for patients
* Support people in their homes, people in care homes and through in-reach in to hospitals where necessary
* Work closely with the community hospitals and specialist teams
Each PCN has a community focused trimuvirate (group of three) of Lead GP / Community; Lead Clinician and Operational Manager. Specialist services and community hospitals work closely with the PCNs to ensure a whole system approach to working across Surrey Downs.

* Dorking Primary Care Network
Dorking community nursing team cover central Dorking & the beautiful surrounding villages of Surrey. Dorking can be described as a charming market town, surrounded by the beautiful Surrey Hills, whilst you are working in the community you can catch a glimpse of the wooded hills and open landscape & as you drive through the streets of Dorking you can experience the historic streets of the town. Dorking is close to the M25 with good aces from Guilford and London. Making Dorking an excellent place to work. Flexible hours can be considered- during the day we cover 8.30-18.30 and have a twilight team from 18.30-23.30hrs. We work closely with the GP practices, Matrons, physio, hospices and Dorking community hospital to deliver interventions such as blood tests, wound care right through to high level complex care with the aim of avoiding hospital admissions.
