Main duties of the job The post will require the completion of detailed health needs assessments and reviews in line with current legislation and local policy regarding Continuing Health Care. There will be extensive liaison with clients, families, care providers and other professionals within the health and social care sectors. The post holder will also be required to provide case management for complex Continuing Health Care patients and their packages of care, making the role varied from day to day. The successful candidates will need to be able to demonstrate the ability to complete comprehensive nursing assessments, have good written and verbal communication skills, must have the ability to work in stressful situations and be able to prioritise and organise their own workloads to meet deadlines.
