Job summary
South Warwickshire University NHS Foundation Trust is looking for a caring, compassionate, self-motivated and reliable Nurse to work in both community hospitals, Nicol Unit based at the Stratford upon Avon Hospital and Campion Ward based at the Royal Leamington Spa Rehab hospital. The Nicol unit is a 20-bed GP run ward caring for patients who require admission prevention, a period of recovery, rehabilitation, reablement, palliative care or sub-acute intervention. Campion is a 25 Bedded GP Led ward providing care to patients that require support in recovery, rehabilitation and reablement.
Main duties of the job
To deliver a high standard of nursing care to patients undergoing surgical or medical treatment. The post holders will be responsible for the assessment of patient care needs, and implementation and evaluation of the programme of the care. The post holders are required to work as part of a multidisciplinary team committed to delivering and improving a high standard of nursing and therapeutic care within South Warwickshire University NHS Foundation Trust.

Clinical
Take clinical responsibility for patients within your allocated area of work.
Continuously assess, plan, implement and evaluate treatment, using appropriate programmes/methods of care.
To ensure a high delivery of quality care in accordance with Trust policies, procedures and guidelines. Participating in activities to improve the quality, productivity and effectiveness of care in response to local and national policies and initiatives.
To practice in accordance with the NMC Code of Professional conduct and scope of professional practice and maintain own NMC registration.
In the absence of a senior colleague, and following appropriate induction and guidance, will be expected to co-ordinate the shift, and take management responsibility for admissions, transfers and discharges to the ward or department.
Liaise with the multi-disciplinary team to ensure an effective flow and communications within the area, including being available for Board and Ward Rounds as required.
Communication
Showing empathy, understanding, and professionalism, when communicating with patients, their carers and families. Identifying individual needs and therefore utilising a range of agreed communication mediums.
Communicate effectively and professionally with colleagues and all levels of staff.
Maintain confidentiality at all times, as required by legislation and Trust policy.
