We are 2 practices based across 2 sites with circa 33,000 registered patients across the north of Portsmouth City. We are looking to recruit an Admin Care Coordinator on a Wednesday and Friday, to join our extensive practice and PCN teams. Our multi-disciplinary team already includes GPs, Nurse Practitioners, Nurses, Healthcare Support Workers, Phlebotomists, Pharmacists, Pharmacy Technicians, Social Prescribers and Physiotherapists. We have excellent communication and team-working.
Main duties of the job
The Admin Care Coordinator will work with the PCN central team supporting the delivery of NHS England's Comprehensive model of Personalised Care and the Early Cancer Diagnosis Direct Enhanced Service. This pivotal role will support multidisciplinary teams by helping to coordinate bi-weekly meetings, and also support the wider PCN team by assisting with general administrative duties that will help promote a positive patient journey.

Portsmouth North PCN is constantly evolving, trying to provide the best care for our patients in the best way possible. You would be joining an existing team of 3 Clinical Pharmacists, 4 pharmacy technicians, two Social Prescribers, a Health & Wellbeing Coach and three Care Coordinators, as well as First Contact MSK Physiotherapists.
Job responsibilities
1. Oversee and co-ordinate bi-weekly MDT meetings; to include invite and agenda sharing, maintaining patient lists and minute-taking and distribution.
2. Support the achievement of key performance indicators, by ensuring patient records are coded correctly in line with the FIT Test referral process.
3. Assist the wider PCN team with any patient-related admin tasks; to include booking patients into clinics, sending communications around appointments and issuing reminders around events/services.
4. Work with the surgeries to ensure all referral waiting lists are kept up to date.
Person Specification
Qualifications
* Educated to GCSE Standard (Grades A-C) or Diploma/HNC level (or relevant experience)
* NVQ in health and social care
* ECDL or equivalent
* Long term conditions training
Experience
* Relevant health and social care experience at AfC Band 3 or equivalent and/or previous experience in the NHS or social care or relevant field
* Experience of administrative duties
* Practical experience of being in a patient-facing role
* Working in a multi-disciplinary setting
* Able to demonstrate a clear understanding of working with confidential information and an understanding of service user confidentiality
* Experience in use of databases
* Experience of working in Primary Care/Voluntary Sector/Community
* Experience of working in a cancer care setting
* Experience in promoting health and health improvement within the community, including health promotion campaigns and signposting
* Knowledge/familiarity with medical terminology
* Experience in end of life care
