Radis Community Care have an opportunity for a Team Leader to join our team based at School Gardens Extra Care Scheme in Stouport on Severn

About the role
As a Team Leader, you will be the first point of contact for residents and staff onsite as well as visiting health professionals. This can include:
- Leading the care shift and dealing with any emergencies that arise on shift
- Creating/updating Care Plans, Risk Assessments and other key documentation
- Auditing documentation such as MAR Charts, Daily Diary Records etc
- Completing staff rotas to ensure each shift is adequately staffed
- Completing quality monitoring checks such as spot checks, carers assessments & reviews
- Liaising with other Health Professions such as Doctors, District Nurses etc.
About School Gardens:
School Gardens is in the heart of the historic Georgian canal town of Stourport and is just a short stroll from Stourport’s thriving town centre with its shops, restaurants and waterside leisure opportunities.
There are 60 beautiful one and two-bedroom apartments available to people aged 55 or over, set amidst a range of communal facilities including a café bistro and lounge, activity studio and a hair and beauty salon.
About You
- Be able to work a mix of early shifts, late shifts, and alternate weekends
- Be willing to take part in a on call rota
- Worked in the Health & Social Care sector for a minimum of 2 years
- Be a caring individual who is passionate about putting the client first and ensuring we provide an ‘Outstanding’ service we would want our own families to receive.
- Have the Right To Work in the UK (Please note that we are unable to provide sponsorship for this role)
Hours:
WEEK 1: Monday, Tuesday, Wednesday, Friday, Saturday & Sunday
WEEK 2: Monday, Wednesday, Thursday, Friday
Shifts - 7am-3pm, 2pm-10pm, and 9am-5pm. This role is mainly office based if no care shifts need to be covered.
