Look no further! We have a fantastic opportunity for a Children and Young People Health and Care Lead to join our team here at Stockton-on-Tees Borough Council. This exciting new role which will allow you to transform the way we work across Health, Education and Social Care, with responsibility as the Designated Social Care Officer (DSCO) and Children's Continuing Care (CCC) Lead. In this role, you will drive the development of integrated support systems across Health, Education, and Social Care, ensuring that the unmet needs of young people are identified early and met effectively. As part of your DSCO duties, you will attend and offer your expert advice at all panels within health, education and social care and act as a specialist resource of information on social care, allowing EHCP plans to progress with good quality social care information. You will have a strong vision and will lead on the transformation of our systems and processes for transition to adulthood for young people with SEND, including those with EHCPs. You will also act as the strategic lead in developing the social care elements of the SEND strategy in our local authority. This role will allow you to use your expertise to make a tangible difference to the social care and SEND provision in the authority, by providing training and raising awareness of CCC and DSCO processes across Stockton-on-Tees.

* Do you have a strong track record of effective service delivery and partnership working?

* Do you have extensive experience of Children's Continuing Care and achieving great outcomes for young people?, A relevant degree in a related subject
* Significant experience in social care, SEND or healthcare and CCC
* An in-depth knowledge of social care, the SEND reforms agenda and the implementation of SEND legislation in practice
* A strong track record of effective service delivery and the design and implementation of robust project plans
