The role We are seeking a Service Delivery Manager to lead and support the Adult Social Care responsibilities for admission avoidance and discharge from hospital. This pivotal role involves overseeing the delivery of high-quality Intermediate Care or Enablement services ensuring seamless coordination between hospital and community care and support. Key Responsibilities Lead and manage teams that support avoidable admission, discharge from hospital and case management to deliver exceptional adult social care services. Ensure effective communication and collaboration between hospital staff, social workers, and community care providers. Implement and monitor care plans to meet the diverse needs of patients transitioning from hospital to home or other care settings. Maintain compliance with relevant legislation, policies, and procedures. Oversee the allocation of resources, including staff and budget management. Maintain accurate records and prepare detailed reports on service delivery and outcomes. Develop and maintain strong partnerships with the Integrated Care Board to enhance service integration and patient outcomes as a key point of contact for ASC. Take on Lead roles across ASC to ensure there is a strategic focus with operational delivery.
