Early Childhood Family Mental Health/Outpatient Case Manager

The Children’s Integrated Services (CIS)/Outpatient Therapeutic Case Manager for Lamoille County Mental Health Services in Vermont will provide case management, psychoeducation, and clinical recommendations to a caseload made up of half children aged 0-6 and half children being served in the outpatient program up to the age of 22, performing 22-25 hours of direct service per week. The case manager works with families to develop treatment plans and implement therapeutic interventions to support clients in reaching established goals across settings, including homes, child cares, and schools. Additionally, the case manager is responsible for coordinating services, collaborating with other service providers, and maintaining appropriate documentation of services provided.
