The Workington Community Rehabilitation Team are looking to appoint an Assistant Practitioner or a candidate who is willing to work towards the Assistant Practitioner qualification. This role will require the successful candidate to work within one's own delegated caseload, making day-to-day assessments of the patients' condition and liaising closely with the ICC MDT and other agencies regarding treatment progression, patient safety, and ongoing care.
Main duties of the job
Provide support and assistance to registered clinicians in the delivery of community rehabilitation and nursing services.
1. Provide support to registered clinicians in the assessment and management of patients in a manner which respects people's privacy, dignity, individuality, and promotes the independence and well-being of patients.
2. Be responsible for a delegated caseload of patients, identified by an allied health professional or registered nurse as being appropriate for the intervention of the post holder. Prioritise own workload dependent on team pressures and dependency of patients.
Use specialist skills and knowledge to provide flexible care for patients with a variety of physical, psychological, social, and environmental needs to enable clients to maintain independence and well-being.
Implement interventions that enable patients to achieve their personal goals, using continuous assessment and adjusting clinical interventions to meet the fluctuating needs of the patient. Evaluate the effectiveness of the interventions.

Person Specification
Qualifications
1. Maths and English GCSE Grade A to C or equivalent/ Grade 4-9
2. Assistant Practitioner Foundation Degree or willingness to study and achieve the Assistant Practitioner Foundation Degree
3. Care Certificate
Experience
1. Experience of working in health and social care setting
2. Experience of working in a community setting.
3. Previous experience of working in a multi-disciplinary team
4. Experience of working within a therapy team environment
Knowledge
1. Understanding of the basic philosophy of rehabilitation.
2. Evidence of ongoing training and mandatory courses.
3. Understanding of reflective practice and its application.
4. Appreciation of the roles of other agencies both voluntary and statutory.
5. Self-awareness and appreciation of limitations.
6. Knowledge and understanding of moving and handling policies and procedures.
7. Understanding and knowledge of relevant National Service Frameworks and guidelines.
Skills and Aptitude
1. Able to undertake work delegated by registered clinician.
2. To be able to seek support from registered clinicians as required.
3. Ability to communicate effectively both verbally and in writing.

4. Ability to work a shift pattern. Monday to Sunday 8:00-20:00.
